A copy of diploma obtained from the one-cycle program and its equivalent academic degree;
2 photos, size 3X4 (cm);
A copy of the military record (for male students);
State certificate in medical specialty obtained as the result of passing unified postgraduate qualification exam;
List of modules passed on other residency programs and certifying information (if applicable);
A document verifying participation in the conference (if applicable);
A document certifying medical experience (if applicable).
